Overlapping comments

To further support content collaboration, Canvas now supports overlapping comments in documents and notes. You no longer need to strategically place comments in a way that doesn't overlap with existing comments.

• Fixes an issue where an HTML table wouldn't get pasted if it only contained table headings.
• Added mitigations for "flash of old content" that could sometimes occur when connection was re-established while editing
